How to Beat Dr. Fetus in Super Meat Boy 3D

Dr. Fetus is the final boss of Super Meat Boy 3D, waiting for you at the end of Visceraville after you've carved through four other bosses across the game's worlds. He's the toughest of the bunch, but here's the thing: if you made it this far, you already have the muscle memory to handle him. The Visceraville levels leading up to this fight are genuinely harder than the boss itself. This guide breaks down all four phases so nothing catches you off guard.

What makes Dr. Fetus different from other bosses?

Every boss in Super Meat Boy 3D is built around memorization and execution rather than reflexes alone. Dr. Fetus follows that same philosophy but layers four distinct phases back to back, each with its own spatial logic. You're not grinding down a health bar. You're reading patterns, positioning correctly, and surviving long enough to reach the next phase. Understanding that framing makes the whole fight less stressful.

Phase 1: Cross lasers and spinning

Dr. Fetus opens the fight with a double bird flip before firing two lasers arranged in a cross shape from his giant robo-hands. Those hands move back and forth along the sides of the roof.

The safest approach is to treat this like a jump rope drill. Alternate between the top-right and bottom-left quadrants of the roof space, leaping as each laser sweeps through your position. It's rhythmic once you lock onto the timing.

After several successful jumps, the cross formation begins spinning counterclockwise while holding its X shape. Stop jumping and simply follow the open space along the ground, moving with the rotation. No special technique needed here.

The phase ends when the fists slam the ground and knock you into phase 2.

Phase 2: Sweeping hands and shockwaves

Two waves of hands sweep across the floor from opposite directions. They do this twice per wave. Neither pass is especially dangerous if you're already comfortable with the platforming the rest of the game has thrown at you. Stay mobile and jump at the right moment.

After both sweeps, the fists start alternating ground punches that send shockwaves rolling across the floor. You need to jump over these, and the key detail here is that you can jump over multiple shockwaves in a single leap at certain points. The game actively encourages this.

The timing is the trickiest part of the entire fight. Watching the video above before attempting this section is worth the two minutes it takes, because seeing how many shockwaves stack under a single jump saves a lot of trial and error.

Phase 3: Electrified floor and wall hopping

You drop down to a lower arena for this one. The moment you land, attach to the left wall immediately. That's the only safe starting position. The floor becomes electrified in alternating grid patterns that switch between walls, so staying grounded will kill you.

From the left wall, hop back and forth between the left and right walls as each grid pattern shifts. Three wall swaps get you through this segment. After the third swap, the floor stops being electrified. Drop down, and the hands carry you back up to the roof for the final phase.

Phase 4: Laser gun and the final sprint

Back on the roof, Dr. Fetus pulls out a laser gun and attacks directly. He always swings the laser to the left first, so stand on the right side of the roof to give yourself the most reaction time. When the laser swings back toward your position, jump rope it.

After surviving the laser, a giant metal ship attaches to the back of the roof. Ride the boost arrows on the right side to get on top of the ship. Once you're up there, the fight is effectively over: sprint straight at Dr. Fetus and leap into him to finish the battle.

What happens after you beat Dr. Fetus?

There's a short cutscene, then one more quick escape sequence with Bandage Girl. It's brief and straightforward compared to everything you just survived. The credits are right on the other side of it.
