Fallout 76 has a new update live today, and the developers are calling Infestations one of the most accessible routes to four-star legendary gear the game has ever offered. The update drops alongside a brand new season, Appalachia Under Siege, and brings a genuinely fresh encounter type that spreads dynamically across the map rather than sitting in a fixed location waiting to be ignored.

What Infestations actually are
The core idea is straightforward: hostile factions and forces seize up to 40 unique locations across Appalachia, and it falls on players to drive them out. Unlike the Burning Springs update, which leaned heavily on new events, NPCs, and map expansions, Infestations are designed to layer on top of what already exists. The goal, as creative director Jon Rush describes it, is to make the map feel alive and constantly active.
"I love seeing all the dots moving around, to me it sounds like the map is 'sizzling' there's stuff going on on the map constantly," Rush said in a group Q&A. "Infestations is another one of those forays into adding another layer of experiences on our map."
The encounters spawn randomly, and players have to locate the specific infestation area themselves. Senior systems designer Fahad Khan confirmed the cap sits at roughly 5 infestations spawning every 5 minutes. If nobody engages, the infestation simply disappears after 5 hours.
The four-star legendary angle
Here's the thing: this is where Infestations get genuinely interesting for players who have been grinding toward better gear. Lead designer Carl McKevitt was direct about it: "It is one of the easiest ways to get a four star."
That matters because four-star legendaries have historically sat behind Raid content, which is demanding, requires coordination, and locks out a portion of the player base. Infestations sit in a middle ground, described as floating "between end-game and casual friendly." You do not need a dedicated group. Anyone on the server can show up and help, making it closer to Fallout 76's organic cooperative moments than a structured Raid run.
That said, Rush is clear that these encounters are not trivial. "If players left at a mid level, maybe wait until you start to see the Infestation get a lot more dots around it before you hop in. These are hard, these bosses are hard, it is end game content to an extent."
Mid-level players should approach Infestations with caution. The boss encounters are tuned for late-game builds, and going in solo without the right perks is a fast way to lose your junk.
The new four-star legendary mods available from Infestation drops include Hauler's, Raging, Satiated, Tarnished, and Vector.
How the loot pool is structured
McKevitt also clarified how Infestations fit into the broader legendary economy. There is a "common pool" of four-star mods, which covers roughly half of what was introduced with Raids. New sources of four-star drops will pull from this common pool alongside an exclusive pool tied to that specific content.
The intent is to avoid making Raids feel redundant. "We want to try and preserve Raids as where you get some of the top tier most highly desired four stars," McKevitt explained, "but getting any four stars is still better than having none."
So Infestations are not replacing Raids as the premium loot source. They are filling a gap for players who are past the early game but not yet running Raids consistently.
Rewards beyond the legendary drops
Beyond the four-star gear, Khan confirmed there are rewards specifically tied to finding and completing Infestations, both in quantity and type. There are no new Xbox achievements or trophies tied to the update at launch, but additional team-based and gameplay challenges are planned for future rollouts.
