How do you get the Skeleton Key in Fortnite?
The Skeleton Key doesn't drop from enemies or appear in regular chests. It comes from buried chests hidden inside the stone mausoleums within the Nightmare Neighborhood graveyard. You need your Pickaxe to dig them up.
There are three mausoleums in the graveyard, but only two of them contain buried chests. That means you can find up to two Skeleton Keys per match across both Zero Build and Battle Royale modes. The item spawns nowhere else on the island, so don't waste time searching outside this POI.

How to reach the secret room in the Nightmare Neighborhood mansion
Once you have a key, head north of the graveyard to the large mansion nearby. Enter through the front, then take the first door on the left. That door leads to a staircase. Go down two flights of stairs to reach the basement.
At the bottom, three doors wait in front of you. This is where the Skeleton Key gets used.

The loot behind each door is randomized every match, so there's no permanently "correct" door. Based on what's been found across matches, here's the general breakdown:
The Rare Chests are the target. They have a chance to contain Exotic weapons, making them worth the trip even if you've already looted elsewhere in the POI.

Shorter answer: yes, if you're dropping Nightmare Neighborhood anyway. The graveyard is a natural looting stop, and the mausoleums are close together. Grabbing both keys adds maybe 30 seconds to your rotation.
The mansion basement run takes a bit longer, but two Rare Chests in a basement with no enemies is a strong trade. Exotic weapons from those chests can swing a match, and the Fortnitemares event makes Nightmare Neighborhood one of the busier drop zones right now, so having great loot early matters.
