Legendary items fall into two broad categories. The first is boss drops, which come directly from defeating vampire boyars in their lairs, inside Greifberg Castle, or occasionally in unexpected locations tied to the main story. The second is rewards, which arrive either as quest completions or from chests hidden across the map.
One thing worth knowing early: don't discard a legendary you picked up at a low level. Blacksmiths and armorers can upgrade any legendary item, so your early finds stay relevant deep into the campaign.
What legendary gear drops from bosses?
Three vampire boyars drop legendary equipment directly on death:
These three are the most straightforward legendary items in the game. Fight the boyar, win, collect the drop. Imbued Sword of St. Mihai
This one takes real effort. You need to complete two connected quest chains: A Monk and a Saint, which has you tracking down the Broken Sword of St. Mihai, followed by Forge It Anew, where you collect three Sword Shards. Once you have everything, bring the pieces to the uriash blacksmith to forge the blade. There's also an optional gauntlet of six enemy waves that empowers the sword further if you can clear it.

Durandal
Head to the broken bridge west of Briar Sloughs to trigger the Cursed Chasm side quest. Drop into the ravine, clear the creatures waiting below, and loot the destroyed cart at the bottom.
Erkas' Sword
This sword ties into the Father Florin quest line. After accessing the ancient ruins beneath Svartrau City, find the strange mural to start the Champion of Old quest. Follow the marker and defeat the miniboss Erkas the Repenter.
Gladius
Once wielded by Brencis himself. Find it inside the Beast of Balaur's lair: defeat the miniboss, then use Shadowstep or Planeshift to scale the wall. The Gladius sits in a chest at the top. The same area contains a Phial of Brencis' Blood, a legendary consumable that teaches the Piercing Shriek ability, so don't rush out after grabbing the sword.

Isbrand's Plate
You pass through this location during Lacra's Hive and Seek quest. Before freeing Isbrand, check the chests on the right side of the chamber after clearing out the kobold horde.
Pieter's Garb
A reward from Volk's Buried Past quest. Complete Volk's tasks and hear out his stories until he reveals the location of your father's treasure. Travel to the secret glade and dig up the chest.
Arbiter's Medallion
From Parlita village in Boar's Back, take the road southeast to the bridge. Spot the waterfall in the distance, walk through it, and loot the hidden cavern behind it.

Runic Enchantment
This piece and the Ancient Greaves both come from Anca's Where Old Devils Lie/Stronger Than Achilles quest chain. Work through the ruins until you reach the trial door, where Anca paints runic markings on Coen's body. The Runic Enchantment is a guaranteed reward at that point in the quest.
Ancient Greaves
Also from the Stronger Than Achilles quest, but tied to a specific choice near the end involving the Font of Life. Your decision there determines whether you walk away with these or not.

The southern half of the map, covering Svartrau City, The Silts, and Tantari Woods, holds some of the strongest late-game pieces.
Ring of Quick Strikes
Travel west along the road near the mountain range by Rockfalls until a quest icon appears. Investigate to pick up the Scholar's Map and Scholar's Key. Head to the small ruins nearby, skip the two uriashi (fighting them isn't required), and use the key on the door.
Wolven Bracers
Follow the blood trail starting from the road near the Woodcutters' Camp shrine. The path leads to a small clearing where wild animals are waiting. Clear them out to claim the bracers.

Ancient Hero's Armor
Found in the ancient temple beneath Svartrau City, accessed during the Hero's End quest from Father Florin. Defeat the miniboss Rigos, the Betrayed Hero to finish the quest and receive the armor.
The Hand of Fate
Also located in the underground temple beneath Svartrau City, but requires extra preparation. Before starting Hero's End, grab the Stone Key from the cathedral treasure. Once underground, find the pit with the green glow, clear the kobolds in the area, and unlock the chest.
Amulet of Crimson Steel

The Vrakhir
Found during The Night of Horrors, which you access by progressing far enough into Lacra's quest line. Move through the tunnels until you spot this sword lodged in the chest of a dead creature. It's hard to miss once you're in the right area.

Located near the southernmost section of Tantari Woods. Follow the sloping path down until you encounter a group of kobolds. Defeat them to unlock the chest.
