Nurse Harrow arrived in Deadlock on October 20, 2026, completing the City Never Sleeps hero roster. She plays unlike any other support in the game: her healing output depends directly on how aggressively she deals damage and collects leeches, making passive backline play a losing strategy. Get the resource loop right and she can sustain teammates through extended fights while terrifying entire enemy teams with Lurking Terrors. Get it wrong and you run dry at the worst moment.

Quick tip: Nurse Harrow sits at A tier with a 49.6% win rate across 8,307 matches at Phantom+ rank as of the October 20, 2026 patch. Her 91.4% pick rate reflects strong early interest, but the win rate suggests there is real skill expression to unlock.

What makes Nurse Harrow tick?

Harrow's kit connects four abilities into one economy. Leech Therapy damages enemies and generates leech stacks that permanently improve her regeneration and fire rate. Healing Needles loads a healing magazine that refills through damage dealt and passive charge recovery from collected leeches. Wretched Vapors buys an ally time by deferring incoming damage, but that debt comes due at the crash. Lurking Terrors is her ultimate: a channeled fear that forces enemies to flee or take sustained Spirit damage.

The mistake most players make is treating these as four separate tools. They are one loop. Leeches fuel charges. Charges enable saves. Saves keep damage dealers alive to generate more pressure. Break the loop and the whole kit falls apart.

Ability breakdown

Leech Therapy

Leech Therapy throws a jar of leeches onto nearby enemies, dealing 23 Spirit damage per second (x0.19 Spirit scaling) while draining 0.65 seconds of buff duration per second. After feeding, the leeches crawl back toward Harrow. Picking them up heals her for the health drained and permanently grants +0.6 health regen and +5% fire rate per leech stack. The base cap is 10 stacks; the T3 upgrade raises it to 18.

Death removes 25% of your accumulated stacks, so a reckless pickup that gets you killed is a net loss even if you grab the leeches first. Cast when enemies advance into your range, then collect during safe windows after they retreat.

Upgrade priority:

  • T1 (1 AP): Leeches drain stamina and restore it to Harrow on pickup.
  • T2 (2 AP): Reduces cooldown by 10 seconds, adds +4 Spirit Power per collected leech.
  • T3 (5 AP): +1m radius, +0.3 Spirit scaling, +8 max leech stacks.

The T2 Spirit Power bonus is significant. At 8 stacks that is 32 bonus Spirit Power; at 10 stacks, 40. That strengthens both your healing output and ability damage simultaneously.

Wretched Vapors

Wretched Vapors has a 40-second base cooldown and applies Spectre Rush to yourself or an ally: +25% speed on all actions for 4 seconds, with incoming damage converted into stored spectres instead of immediate health loss. When the rush ends, Spectre Crash applies a 20% action slowdown for 4 seconds and deals all stored damage plus a 30% penalty.

The critical mechanic: incoming healing during the rush removes stored spectres before restoring normal health. That means you need Healing Needles ready before you cast Vapors, not after. A health bar that stays low during the rush is not a sign your needles failed; it means you are clearing the debt first.

Upgrades:

  • T1 (1 AP): -15s cooldown, +7m cast range.
  • T2 (2 AP): +1s rush duration, +20% healing effectiveness.
  • T3 (5 AP): +15% rush speed, dispels non-ultimate debuffs on cast.

The T3 dispel is situationally powerful. Against teams with removable debuffs that repeatedly decide fights, moving Wretched Vapors T3 earlier in your upgrade order is worth considering.

Healing Needles

Healing Needles loads a full clip of syringes that heal allies on hit for 25 health per shot (x0.2 Spirit scaling base). Headshots add a 15% healing bonus, but landing the shot matters more than chasing headshots when an ally is near death. Reloading spends a charge for a fresh clip; putting the needles away refunds unused syringes as a partial charge.

Charges recover two ways: dealing weapon damage, and passive recovery at 0.15% per second per leech stack. That second source is why leech collection matters even when you are not actively healing anyone.

Upgrades:

  • T1 (1 AP): +1 max charge.
  • T2 (2 AP): +20 heal per shot, shots restore 0.6 stamina to the ally hit.
  • T3 (5 AP): +0.4 Spirit scaling, shots extend non-ultimate buff durations by 6% up to their maximum.

The T2 stamina restore is underrated. It gives a fleeing ally two extra dashes worth of escape resources, which can be the difference between a clean disengage and a death.

Lurking Terrors

Lurking Terrors has a 200-second base cooldown. Harrow opens her bag and channels Spirit damage at 60 DPS (x0.3 Spirit scaling) in a frontal arc. Enemies looking at her build terror over 1.3 seconds; at max terror they flee in fear for 2.2 seconds base, losing stamina and movement abilities throughout. Each collected leech adds 0.1 seconds to the fear duration. Enemies facing away take 50% damage unless already feared.

The final upgrade (T3, 5 AP) adds 3 seconds of channel time, +0.5 Spirit scaling, and damage immunity while channeling. That immunity does not grant control immunity. A stun still interrupts the channel.

Early game item order

The standard opening: Extra Regen, then Extended Magazine, then upgrade Extra Regen into Healing Booster, then upgrade Extended Magazine into Titanic Magazine, then Extra Spirit into Improved Spirit, then Warp Stone.

If your lane partner needs immediate healing support, buy Healing Booster before completing Titanic Magazine. If enemies are threatening kills every time you approach the wave, interrupt this order for a defensive item. Dying twice delays every subsequent purchase more than any single item saves.

Midgame priorities

After the opening core, both Duration Extender and Compress Cooldown should imbue Wretched Vapors, since it is your most flexible tool offensively and defensively. Surge of Power bridges your Spirit investment while you work toward the Ricochet and Healing Tempo power spike.

Fortitude is the standard tankiness option to reach the 4,800 Vitality souls threshold. Healing Nova or Rescue Beam work as alternatives if your team needs additional healing output.

Late game

Ricochet works on Healing Needles and synergizes with Healing Tempo, letting bounced healing shots also trigger the fire rate and movement speed buff. That combination is the core of Harrow's late-game support power.

Healing Nova and Restorative Shot both activate Healing Tempo on Harrow herself, granting the fire rate needed to land more healing shots faster. Keep that in mind when choosing between healing options.

For defensive adjustments: Spirit Resilience against concentrated Spirit damage, Dispel Magic for removable debuffs on yourself, and Unstoppable specifically to protect Lurking Terrors casts.

Ability upgrade order

The highest win rate ability sequence across 1,142 matches at Phantom+ is: unlock Healing Needles first, then Lurking Terrors, then Wretched Vapors, then Leech Therapy (50.2% win rate). The second most played order unlocks Lurking Terrors first at 48.7% across 927 matches.

For a beginner support path, the following upgrade sequence prioritizes immediate healing:

StepUpgradeAP CostTotal AP
1Healing Needles T111
2Healing Needles T223
3Leech Therapy T114
4Leech Therapy T226
5Wretched Vapors T117
6Wretched Vapors T229

9Lurking Terrors T1120
10Lurking Terrors T2222
11Lurking Terrors T3527
12Wretched Vapors T3532

Move Wretched Vapors T3 earlier when removable debuffs repeatedly decide fights. Move Lurking Terrors T3 earlier when coordinated objective fights need control more than additional sustain.

How to play Nurse Harrow

Lane phase

Harrow has no base health regeneration, so Extra Regen is not optional early. Avoid prolonged trades; her gun damage is modest and her early sustain is limited. Secure last hits, stay near cover, and throw leeches when opponents advance. Heal your lane partner reactively rather than preemptively burning charges.

If leeches land behind an enemy Guardian, leave them. Dying to retrieve one pickup loses more than the stacks are worth at that point in the game.

Teamfight positioning

Arrive to fights with a full Healing Needles charge. Find a position that gives you a clear firing lane to your primary healing target without placing you in the same area-of-effect zone as them. Cover should protect you, not block your shots.

The priority decision in any fight:

  • Heal when an ally is under focus fire or carrying stored Vapors damage.
  • Attack when allies are stable and you need charge recovery.
  • Reposition when an enemy flank is about to cut off your support angle.
  • Use Lurking Terrors when enemies are committing through the space in front of you.

Tell teammates when your charges are low. A teammate who knows you are dry can disengage; one who does not know will keep fighting expecting a save that cannot come.

Managing the leech economy

Target 8 to 10 leech stacks through safe pickups. At 8 stacks with T2 upgraded, that is 32 bonus Spirit Power feeding into your healing and ability damage. Collect after enemies retreat or your team controls the ground. Abandon pickups that require crossing an exposed firing lane.

Counters and difficult matchups

Who counters Nurse Harrow?

Baba, Apollo, and Ivy are her most dangerous counters at Phantom+ rank. Her best synergy partners are Baba, Wraith, and Rat King.

For lane matchups, three heroes create specific problems:

  • Wraith: Telekinesis creates an opening for burst damage before you can respond. Avoid isolated side-lane duels and keep teammates close enough to answer.
  • Mo & Krill: Their approach punishes an exposed support. Back away before they reach you rather than relying on a last-second heal.
  • Bebop: Grapple Arm pulls you out of your support position. Use solid cover and avoid predictable peeks while the pull is available.

How to fight against Harrow

Force her to choose between self-preservation and supporting her teammate. The specific steps:

  1. Apply Toxic Bullets or another anti-heal item to the target your team is focusing.
  2. Attack from two directions so she must move or lose sight of her patient.
  3. Use a disable when she steps out to heal, giving teammates a window to finish the kill.
  4. Track Wretched Vapors and maintain pressure through the target's crash window.
  5. When Lurking Terrors starts, turn away and leave the frontal area rather than staring into the fear buildup.

Anti-heal is the most reliable counter. It reduces Harrow's ability to clear stored Vapors damage and sustain teammates simultaneously, collapsing the resource loop that makes her effective.

FAQ

Can Healing Needles heal Harrow herself?

No. Healing Needles only heals allies. Harrow's own sustain comes from collected leech stacks, Extra Regen, and other recovery items.

Does Ricochet work with Healing Needles?

Yes. Bounced shots heal nearby allies with reduced healing on secondary hits. The bounce also triggers Healing Tempo, which is why the Ricochet and Healing Tempo combination is so strong in late-game teamfights.

Is Lurking Terrors uninterruptible at T3?

No. The T3 upgrade grants damage immunity while channeling, not control immunity. A stun still ends the cast. Activate Unstoppable before channeling to protect against this.

How many leech stacks should you aim for?

Target 8 to 10 stacks as a working baseline. With Leech Therapy T2, that is 32 to 40 bonus Spirit Power in addition to the regeneration and fire rate benefits. The T3 upgrade raises the cap to 18 stacks for players who can consistently maintain a high count.

What is the best lane partner for Nurse Harrow?

Wraith and Mo & Krill both pair well. Wraith benefits from healing during committed firing windows; Mo & Krill appreciate sustain after emerging from controlled engagements. The pairing works best when you agree on retreat directions before the fight starts, not during it.
