Dying to a boss with its health bar nearly empty is one of gaming's most infuriating experiences, and Crimson Desert dishes it out regularly. Palmar Pills exist specifically to soften that blow: pop one on death and you revive at 30% health, buying you another shot at the fight. The problem is they feel scarce until you know where to look. This guide covers the recipe location, every crafting ingredient, where to find pills in the open world, and the fastest known farming route.
What do Palmar Pills actually do?
A standard Palmar Pill triggers an instant revive when you die, restoring 30% of your maximum health. You can follow that up by eating food for an additional top-up, which makes the combination genuinely useful rather than just a last-gasp measure. Enemies can still attack you while Kliff is getting back on his feet, so be aware of positioning when the revive animation plays.
There is also an upgraded version: the Refined Palmar Pill, which revives you at full health instead of 30%. That variant requires completely different ingredients (covered below) and is locked behind later campaign progress.
Enemies do not pause their attacks during the revive animation. If you get hit again before standing up, the pill is wasted and you die anyway. Use Palmar Pills in boss fights when you have at least a moment of breathing room.

Alchemy Formula: Palmar Pill
Where is the Palmar Pill recipe in Crimson Desert?
The Alchemy Formula: Palmar Pill is inside Shadow's Whisper Cave, located near Three Saints' Falls in the Hernandian Territory, just north of Hernand. You pass within a short distance of it during the opening hours of the game.
How to reach Shadow's Whisper Cave
Here is the step-by-step route from Hernand:
- Head to the Stable just outside Hernand Town.
- Follow the path north toward the large bridge you crossed during the prologue.
- Near the bridge, look for a Shrine just off the path to your right.
- From the Shrine, drop down the ledge to the ground below.
- Travel west along the base of the cliff face.
- You will pass through a hidden crevice that opens into Shadow's Whisper Cave.
Alternatively, Shacknews notes you can spot the cave entrance by cutting right off the road as you cross the bridge and climbing down the cliff face directly.
Inside the cave, hang right and you will find a desk with the recipe scroll and two Palmar Pills sitting next to it. Pick up the scroll and examine it to learn the formula. On the opposite side of the room there is also a Cauldron you can use immediately to start crafting.
There is a second, unmarked Alchemy building with a Cauldron just west of the Tannery in Hernand Town, which is often more convenient once you are moving around the city regularly.
Second recipe source: Pororin Village
According to GameSpot's guide, there is a second way to obtain the formula. In Pororin Village, an NPC named Lumiel runs a research project called Pharmacological Research on Medicinal Herbs. Completing it rewards you with the Palmar Pill formula as well. This is useful if you somehow missed the cave, though the cave route is far faster for most players.
How to craft Palmar Pills
The recipe is flexible. You need 2x Water plus 15 of any qualifying Medicinal Herb. Water costs 3 Copper from vendors including the Grocer's Shop and the Inn, so it is essentially free. The herb requirement is where things get interesting, because the list of accepted plants is long.

Each craft produces one Palmar Pill, so you will need to grind if you want a meaningful stockpile before a tough boss.
Refined Palmar Pill recipe
The Refined Palmar Pill recipe is found much later, in an Abyss island called Ascetic's Rest, accessible after completing Chapter 9: The Sage of the Desert. Its crafting requirements are entirely different:
Palmar Leaves and Palmar Beetles are found in a grove just north of Pororin Village. The Refined variant fully restores your health on revive rather than just 30%, making it substantially more powerful for late-game boss encounters.
Where to find Palmar Pills
Before you have a reliable crafting setup, scattered pills across Pywel can keep you stocked. They glow with faint green sparkles, making them visible if you are nearby and paying attention. According to Polygon, the earliest confirmed locations are:
- Shadow's Whisper Cave (2 pills on the desk with the recipe)
- Specter's Hideout
- Atop the bell tower in Hernand
IGN's wiki notes that pills also appear on altars and gravestones throughout the open world, inside hidden Treasure Chests, and as rewards for completing certain quests. Confirmed quest rewards include the Woman in White quest in Chapter 1 and the Reunion quest in Chapter 2.
Open-world pills respawn but are limited in total number at any given time, so crafting remains your primary long-term supply.
The green sparkle effect on Palmar Pills is subtle. If you are sprinting past altars and gravestones during exploration, slow down. Several pills sit in plain sight but get missed because players are moving too fast to notice the glow.
How to farm Palmar Pills
IGN's wiki team documented a farming strategy combining Rosemary theft in Hernand with Peony foraging along the roadsides, capable of producing at least 11 to 12 Palmar Pills in roughly 20 minutes. Here is how it works.
Step 1: Get a Mask and some Keys
Before entering Hernand to steal Rosemary, you need:
- A Mask: Available from Back Alley Merchants (the one southeast of the Church Confessional Vendor sells them for 10 Copper). You can also loot masks from Bandits or earn one by turning in the bounty on Jeffrey, an outlaw in Hernand.
- Keys (optional but helpful): Purchasable from Back Alley Shops for 30 Copper each, or looted from enemies. Having keys opens extra buildings and meaningfully increases how much Rosemary you collect per run.
Step 2: Farm Rosemary in Hernand
Equip your Mask and work through the northern, southern, and eastern areas of Hernand Town. Enter every building you can, including vendor shops. Rosemary bunches hang from walls and sit near doors inside most buildings. Expect 3 to 5 bunches per building, with some containing upwards of 10.
If you prefer to buy rather than steal, Rosemary is available for 8 Copper each around the Provisioner's Shop, Grocer's Shop, and Tannery.
After stealing, stay inside buildings until the crime meter clears. Your Faction Contribution takes a minor hit, but a quest or two brings it back. In testing, this route yielded around 100 Rosemary in just over 13 minutes, enough for six or seven Palmar Pills.
Rosemary takes approximately two in-game days to respawn, so move on to Peonies while you wait.
Step 3: Gather Peonies while Rosemary respawns
Peonies are the bright pink flowers common throughout Hernand and its surrounding roads. No stealing required. Head to the Stables outside Hernand and forage the Peony patches lining the roadsides:
- Northwest route: Follow the road all the way to the bridge.
- Southwest route: Follow the road until the patch opposite the Bank.
This method produced around 83 Peonies in seven minutes in IGN's testing, translating to approximately five more Palmar Pills.
