Overview
Plague Inc: Evolved is a strategy simulation game where the goal is total human extinction. Starting with a single infected host, players evolve their pathogen by spending DNA points on transmission vectors, symptoms, and resistances while humanity races to develop a cure. The tension between staying hidden long enough to spread and becoming lethal enough to kill before a vaccine arrives is the core loop, and it never gets old.
Ndemic Creations released the game in September 2015 as the expanded PC and console version of the original mobile Plague Inc., adding multiplayer, a scenario creator, and significantly more content. It carries a 4.44-star rating from over 4,100 PlayStation players, which says a lot for a game built around wiping out the species doing the rating.

Gameplay and mechanics
The strategic depth here goes well beyond picking a nasty symptom and waiting. Every decision has cascading consequences across a global map that tracks infection, death, and cure research in real time. Key mechanics include:

Plague Inc: Evolved content image
- 12 distinct disease types (bacteria, virus, fungus, parasite, and more)
- DNA point economy for evolving traits
- Country-specific infection variables like climate and healthcare
- Real-time cure research counter
- Symptom and transmission upgrade trees
Each disease type plays differently enough that the game effectively offers 12 separate strategic puzzles. A fungus spreads slowly and requires deliberate spore burst investments. A neurax worm can literally take over governments. The variety keeps the game fresh well past the first dozen hours.

What makes Plague Inc: Evolved different from other strategy games?
The inversion of the standard strategy game premise is the obvious answer, but the more interesting one is the simulation fidelity underneath it. The global map pulls from real-world geography, population density, climate zones, and travel routes. Greenland's isolation is a running joke in the community for good reason: landlocked or island nations with low traffic genuinely resist infection, and you have to plan around them. That level of systemic authenticity makes the game feel less like a puzzle and more like a model.

The scenario creator adds another layer entirely, letting players build custom outbreak scenarios and share them through Steam Workshop. The community has produced thousands of scenarios, from historical pandemics to fictional outbreaks, extending the game's lifespan well beyond its base content.
Multiplayer and replayability
Plague Inc: Evolved supports both competitive and cooperative multiplayer. In competitive mode, two players race to infect the world with their own pathogens simultaneously, which creates a genuinely different strategic problem since you have to account for another plague competing for the same hosts. Co-op mode flips the script, tasking players with working together to stop an AI-controlled outbreak, which is also available as a standalone DLC expansion called The Cure.
Between 12 disease types, multiple difficulty settings, the scenario creator, Steam Workshop content, and two multiplayer modes, the replayability is substantial. No two runs play out identically because the simulation responds dynamically to your choices, and small early decisions compound into wildly different late-game situations. For fans of biological strategy games and simulation-driven decision-making, that unpredictability is exactly the point.
