Season 15 of Diablo IV packs in one of the most ambitious easter eggs the series has ever attempted: a hidden boss arena where you fight Diablo, Baal, and Mephisto simultaneously. Getting there requires hunting down 10 scattered journals, transmuting three rare Splinters into a single coin, and then surviving a fight that demands Torment 12-level gear. This guide walks through every step, from the first journal to the Hellfire Torch sitting in your inventory.
How do you unlock the Echo of Tristram?
Before any of the portal steps matter, you need to complete the Season 15 main questline. Doing so makes the three Prime Evil Splinters available as drops, one from each of the season's major bosses.
Splinter
Source Boss
Splinter of Terror
Echo of Diablo
Splinter of Destruction
Echo of Baal
Splinter of Hatred
Echo of Mephisto
All three bosses must be fought at Torment 10 difficulty inside their respective Waking Nightmare dungeons. One important note: all three Splinters can apparently drop from any of the three brothers, meaning you can farm a single boss repeatedly if one feels more manageable. Whether that behavior is intentional or will be patched is unclear, so take advantage of it while it lasts.

Once you have all three, open the Horadric Cube, place the Splinter of Terror, Splinter of Destruction, and Splinter of Hatred inside together, and perform a Transmute. The result is Wirt's Lucky Coin, the key item for the entire sequence.
With Wirt's Lucky Coin secured, the next task is collecting all 10 Deckard's Journals spread across Sanctuary. There is no required order. A hidden seasonal quest may trigger automatically after finding a few, but you can pick them up in any sequence without breaking anything.
tip
Journals appear as a small colored blip on your minimap when you get close enough. If you are struggling to pinpoint one, slow down and watch the minimap edges rather than scanning the ground visually.
Here is every location:
Gea Kul (Kehjistan) — on one of the piers west of the Gea Kul waypoint, near the water's edge.
Tarsarak area (Kehjistan) — on the path leading northeast toward the Vault of the Crucible dungeon.
Imperial Library (Kehjistan) — just south of Deckard Cain inside the library itself. You will return here later, so grab it on your first visit.
Kyovashad area (Fractured Peaks) — in the bedroom of Lorath's hut, southwest of Kyovashad.
Yelesna area (Fractured Peaks) — northwest of the Yelesna waypoint, near the entrance to the old Horadric Vault.
Marrowen area (Scosglen) — on one of the piers west of the Marrowen waypoint, along the coast.
Braestaig area (Scosglen) — near the entrance to the Penitent Cairns, northeast of the Braestaig waypoint.
Cerrigar area (Scosglen) — inside a ruined tower outside the Fiefdom House, east of Cerrigar.
Wenjinhani (Dry Steppes) — inside the Alchemist's Hut in Wenjinhani.
Hidden Overlook area (Dry Steppes) — on the broken bridge west of the Hidden Overlook waypoint.
After collecting all 10, your quest log updates to confirm completion. Head back to the Imperial Library in Kehjistan and speak with Deckard Cain. A portal opens nearby. Step through it to reach Old Tristram.
How do you open the Echo of Tristram portal?
Old Tristram has additional NPCs and side content, including a quest involving a sign and a potential encounter with The Butcher. Neither appears to be a prerequisite for the Echo of Tristram event, so you can head straight to the Fountain in the center of the area.
With Wirt's Lucky Coin in your inventory, interact with the Fountain and submit the Coin. A portal to the Echo of Tristram should appear.
If the portal does not appear after the hotfix, try the Solo mode workaround before farming three new Splinters. The farming loop to replace a lost Coin is not short.
The good news: after completing Echo of Tristram for the first time, the three Splinters become available to farm again, so the entire sequence is repeatable.
Diablo 4 Guide: How To Unlock Echo of Tristram & Beat the Uber Prime Evils
How do you defeat the Uber Prime Evils?
Entering the Echo of Tristram drops you into a fight against Echoes of Diablo, Baal, and Mephisto all at once. This is not a fight to walk into casually.
The three bosses deal overlapping AoE attacks that quickly saturate the arena. Standing still to maximize damage output is a reliable way to die. The fight rewards builds that can keep moving while maintaining consistent damage rather than ones that require a stationary rotation.
tip
Whittle all three bosses down to similar health levels before going for kills. Bosses have been observed reviving and rejoining the fight if eliminated too early while the others are still at high health. Keeping them close together in percentage terms before the final push reduces the risk of a revive cascade.
The minimum benchmark for attempting this fight is being able to comfortably clear Torment 12 content. If your character is still working through Torment 10 or 11, the Uber Prime Evils will likely outlast you. Key combat tips:
Save mobility skills and damage reduction cooldowns for moments when two or more bosses stack AoE attacks simultaneously.
Do not tunnel on a single target. Spread damage evenly until all three are near death.
Ensure defensive stats and resistances are fully optimized before entering. The fight punishes undertuned builds quickly.
What rewards does Echo of Tristram drop?
Defeating all three Prime Evils awards the Hellfire Torch, a Mythic Charm that provides bonuses to stats, resistances, and skill ranks. It also increases your set count and allows your character to equip one additional Unique Charm. That last effect alone makes it one of the more impactful items in Season 15.
Beyond the Hellfire Torch, the fight drops high-tier gear. Players have reported obtaining multiple Mythic items and a Banner of Heroes in a single clear, though drop quantities vary between attempts. Do not treat one strong result as a guaranteed baseline for every run.
